Include at least the U.S. Census, National Center for Health Statistics (NCHS), National Health and Nutrition Examination Survey (NHANES), National Immunization Survey (NIS), National Survey of Family Growth (NSFG), Behavioral Risk Factor Surveillance System (BRFSS), Healthcare Effectiveness Data and Information Set (HEDIS) and Guttmacher Institute in your inventory. - Column 6: Add any pertinent information; you may want to identify whether this database is private or government.
