Complete Exercise 11-2 on page 432 and Exercise 11-3 on page 433 in your text, Understanding Health Insurance A Guide to Billing and Reimbursement 13th edition:  Use the blank PDF CMS-1500 Form for this assignment: Blank_CMS-1500-2.pdf   Use the same form for Exercises 11-2 and 11- 3. Download the blank PDF again and use it for Exercise 14-2.  (If you have technical issues saving the edited form, be sure you download the latest Adobe Acrobat Reader.)   1, Exercise 11-2: You will enter procedures in Block 24 of a CMS-1500 Claim Form. 2. Exercise 11-3: You will complete Block 33 with the information provided.

3. Exercise 14-2:

You will complete the Mary S. Patient Medicare Primary CMS-1500 claim (Exercise 14-2 on page 530). Download the PDF claim form above (you won’t need to use the claim form in the Appendix as mentioned in #1 of exercise) and save it as “YourNameMarySPatient” when completed. This exercise has specific instructions, so please follow them carefully. Compare your claim with the completed claim in Figure 14-8 on page 531. Your claim should look like that before you submit it. This is a great low-stress opportunity to successfully complete a Medicare claim form!
