  • Use only the Alphabetic Index of the latest PCS code book for this. Do not use the coding tables in this Lab Activity as that will be done in the next Lab Activity called Practicing Coding Pathways.
  • Below you find a list of 10 inpatient procedures to look up in the Alphabetic Index of the ICD 10 PCS code book. You will look these up either by the name of the procedure or the Root Operation. Decide on the Root Operation first and look up the code based on the Root Operation if possible. If not, then try looking up the procedure.
  • Use your Root Operation guidelines in your code book found in the front of your code book and the Root Operation descriptions found in your appendixes in the back of your code book.
  • Remember that in PCS coding you will build your code using the Coding Tables located after the Alphabetic Index. The Alphabetic Index does not always give you the full code. Often only the first three or four characters. Sometimes you will see the whole code. If the whole code is given, still double check in the Coding Tables.
  • Find the most specific first three characters based on your documentation.
  • Enter your three characters next to the number that corresponds to the procedure you are coding in a Word Document and submit it to the drop box.

Below are the PCS procedures to look up in the Alphabetic Index:

  1. Manually assisted vaginal delivery of a newborn with no complications
  2. Amputation (detachment) of the left ring finger, complete
  3. Drainage of the right pleura cavity by thoracoscopy with fluid used for diagnostic purposes
  4. Gastrectomy, partial of the stomach via an open approach with no diagnostics done
  5. Inspection of the upper intestinal tract via endoscopic approach via the mouth for purposes of diagnosis. All findings normal
  6. Thyroidectomy of the whole left lobe (resection) by open approach
  7. Cholecystectomy of the whole gallbladder (resection), open approach
  8. Total left knee replacement, open approach, using a synthetic substitute and cement
  9. Immobilization of the right finger external approach with a splint
  10. CT (Computerized Tomography) of the chest and abdomen with no contrast

TIP: Be sure to double check your use of the Root Operations Excision and Resection (found in the Appendixes in the back of the code book) and know the difference when looking up the code in the alphabetic index. One is for removing the whole body part and one is for removing part of the body part.

2nd Half of Assignment

  • Use the three characters from your Alphabetic Index Lab Activity and the Coding Tables found in the code book after the Alphabetic Index.
  • Below you will find the same list of 10 inpatient procedures you used in the previous Lab Activity called Alphabetic Index..
  • Remember that in PCS coding you will build the last 4 characters of your 7 character code using the Coding Tables found after your Alphabetic Index. You will be coding from the Medical and Surgical Section Coding Tables only.
  • Find the Coding Table that is identified by the first three characters you found in the Alphabetic Index. These are in numeric and alphabetic order. You will see the indicators on the outer band of the pages.
  • Once you find the correct Coding Table, choose a character option from each of the four columns so that it matches your documentation.
  • You can only choose one character choice per column.
  • Use your information in the front of your code book and the appendixes in the back of your code book to understand how to choose the last four characters which are body part, approach, device, and qualifier. This information is found in the Medical and Surgical section introduction and guidelines. Do not use any of the other sections for coding.
  • Place your code (1 code per procedure) next to the correct number on a Word Document and submit to the drop box.

TIP: Use your appendixes in the back of the code book if you are not sure about which body part, approach, device, or qualifier to pick in your Coding Table. The appendixes contain definitions, in depth explanations, and examples. Read in the front of your code book the definition of a device and a qualifier.
