Preparatory work
¢ Identify a personal health behaviour that you have considered changing to improve your own health, i.e. you would like to stop smoking, have a healthier diet, take more exercise, etc and prepare a plan of action for change
¢ Over a period of a month, carry out this plan keeping a reflective diary on your journey each week identifying potential /actual threats to your success and what has helped you achieve success.
You should include in the main body of your assignment (1500 words):-
¢ An introduction detailing the purpose of the assignment and the chosen health behaviour. Identify the detrimental effects (or risks) that this health behaviour has on the body and person.
¢ Include contemporary evidence from peer reviewed journals and national guidelines to support the need for this behaviour change
¢ Identify the health benefits of changing this behaviour would make. Describe and analyse behaviour change theory and apply this to your chosen health behaviour change
¢ Identify factors that have helped or prevented you from succeeding and reflect on how this change has (or has not) impacted on your health detail from your reflective diary can be aIDed in an appendix, thereby saving on your word count.
¢ Your work must be supported by current, evidenced based references.
