The main body –
The main body should include 3 sections
1. A brief outline of the chosen long term condition including pathophysiology and incidence. (approx. 200 words)
2. An in-depth exploration and discussion of the Health care professionals’ role in providing holistic practice at one stage of the patient journey. This can be either, diagnosis, living for today, progression/ transition or end of life. Discussion must be supported with reference to current literature and evidence based practice guidelines. (approx. 1500 words) (Diagnosis Stage to be discussed).
3. A brief exploration and discussion of the Health care professionals’ role in providing holistic practice throughout the other three stages of the patient journey. This section should only include the key issues for your chosen condition. (approx. 250 words for each of the 3 stages)
Conclusions and Recommendations –
This is a summary of the key points that you have raised within the essay. Demonstrate that you have explored the patient journey as set out in the introduction. Identify how the new learning gained will inform your future practice in relation to providing a holistic approach to long term conditions. (approx. 350 words)
