The content areas what would apply to me is special populations. In my current practice as a case manager, I plan discharge with patients who often require alternative plans. Most of the patients are elderly and have multiple medical problems. Also most have issues such as polypharmacy. The 3 learning goals I set for myself during the course directly relate to this learning module: Explain the steps in planning and evaluating health care programs Identify determinants affecting the health of the elderly. Analyze the role of population health nursing with respect to the elderly.
