Diet Diary. You can use the suggested food diary sheet to keep track of your dietary intake. You can also use your phone and take photos of your meals. Follow the directions for keeping accurate records. The more complete your diary appears, the better your analysis will be! This is for you! I grade so many of these I wil never remember what diet an individual has!

Suggestions for filling out the form: Bring a 8 oz cup (be sure of the size) along with you and estimate how much food you are eating. If you enter 1 cup you also need to enter number of servings (example 1 serving of 1 cup).

DIET INTAKE RECORDING DIRECTIONS:

Pick a “usual day” for your food intake record! Do not pick a very different day for your assignment. You want to see what you eat on a typical day!

NOTE: WE DON’T USE SUPPLEMENTS IN THIS ASSIGNMENT!

Write down EVERYTHING (food, drink, supplements) that you eat. Do not put supplements or water into the program. Include candy, condiments, dressings, alcohol (if you consume this), etc. In addition to recording everything that you eat, estimate the amounts (i.e., portion or serving size) of the food or beverage that you consume. This can be done by bringing a measuring cup with you and estimating how many cups of a food you will eat. For combination foods (i.e., sandwiches), you can try to find the food in the database, or you can break the food down into its component parts and list them separately:

For Example: A Tuna Sandwich:

Bread, whole wheat2 slices (Note weight of slices)

Tuna, canned with water0.25 cup

Mayonnaise, regular 1 tablespoon

Pickle 1 large dill

Other suggestions –

1) For as many foods as possible, copy the name brand.

2) Carry the recording sheet with you so that you don’t forget foods you eat. Put them in a page protector to keep them in decent condition.

3) Record the approximate time the meal/snack was eaten and what is was (i.e. breakfast, lunch, snack, etc.)

4) Record the amounts of all food and drink consumed in common measurements:

Bread – slices

Cereal/Grains/Pasta/Beans – cups, dry ounces

Cereals – record product name!

Beverages – cups, ounces

Meat/Fish/Poultry/Cheese – ounces, pounds, slices

Veggies/Fruit – whole: small, medium, large; cups

Salad –List what is in the salad or use the listing for mixed salad. For tomatoes, measure the size of the slice or whole tomato if you use one.

Condiments/Dressings (sugar/mayo/ketchup/peanut butter/jam) – packets, tablespoons, teaspoons

5) For beverages-

Milk – specify if non-fat (skim), low-fat, whole, chocolate, soy, cream, etc.

Fruit Juice – specify if fresh or frozen, indicate percent juice (e.g., 5%, 100%, etc.)

Water – Do not keep track of water drunk – can vary widely!

Coffee/Tea – specify if added milk (type), sugar, etc. For specialty coffee/tea drinks indicate if made with whole, low-fat or non-fat (skim) milk and if with whip topping, as applicable. Copy kcal from shop literature!

6) Breads-Specify type: whole grain, 9 grain, white, rye, low-carb, etc, and toppings (margarine, jelly, etc.)

7) Desserts: Specify brand name (if packaged item) or homemade; Specify if low in fat, sugar, etc.

8) Snacks- Try to eat a multiple of the serving size listed on the food label. This is to make it easy to put into the program.
