Integrated Health Promotion Plan

1. The term “integrated health promotion” refers to working in a collaborative manner with the client and the interprofessional team using a mix of health promotion interventions and competency-building strategies to address priority health and well-being issues to achieve quality patient outcomes.
Using the information you gathered during the Health History (M1A1) and Genogram (M3A1) assignments, you are now ready to create an integrated health promotion plan for the same individual.
Follow these instructions for completing the Integrated Health Promotion Plan. This assignment accounts for 25% of your course grade. Use the Integrated Health Promotion Plan Scoring Rubric for this assignment as a guide for writing your plan.
For this assignment:
1. Introduction: Write an introductory paragraph that includes the purpose, focus, and scope of the assignment.
2. Section One – Identification of Issues: Identify at least 2–3 health promotion and/or disease prevention priority issues related to the client’s real or perceived health problems or family risk factors based on the health assessment data collected for the Health History (Assignment I) and Genogram (Assignment II). To accurately identify the issues at hand, the nurse must be cognizant of the client and fully familiar with his/her health information. Thus, be sure to also
a. Identify the client and situate him/her as an individual and as an individual within a family, and
b. Clearly and accurately summarize the Health History and Genogram.
3. Section Two – Health Promotion Plan: Develop a caring, holistic, integrated, and culturally-competent health promotion plan that
a. Reflects an accurate and full understanding of the client (development, genetics, history, health beliefs and practices, heritage) and collaboration with the interprofessional team and client’s health issues (as identified in Section One), and
b. Includes 2–3 accurate and appropriate wellness interventions for each issue identified in Section One. Each intervention should have a corresponding Nursing Interventions Classification (NIC).

4. Section Three – Evaluation of Plan:
a. For each issue identified in Section One, identify at least one expected outcome that will result from your Health Promotion Plan. Each expected outcome should have a corresponding Nursing Outcomes Classification (NOC).
b. Describe how you will evaluate the effectiveness of your Health Promotion Plan (e.g., have client come back in 3 weeks and weigh or calculate another body mass index). Evaluation should be specific, relevant, and reasonable.
5. Section Four – Research Integration: Select at least three current peer-reviewed journal articles (published within the past 5 years) that support the selected health issues or risks, identified interventions, and expected outcomes. At least two articles must be from nursing.
a. Explain the supporting rationales or evidenced-based practices from each article as it relates to your client.
b. Discuss how the article information contributes to nursing knowledge in the areas of health promotion and disease prevention.
6. Conclusion: Clearly summarize all the key points.
The Introduction and Conclusion should be in narrative form. However, you can create a chart or graph for Sections One through Four, but write in complete sentences.
This plan is limited to 5–8 pages (excluding the title page and references) and must reflect APA standards for formatting (e.g., title page, headings, references, grammar, punctuation, mechanics, usage) and writing consistent with Standard American English.
