Synopsis of the Shadow Health Assignment
The Shadow Health assignment involves Rachel Adler, a 16-year-old who came to the clinic for her physical assessment. The patient noted that she has never been ill before and has no health problems currently. She notes that she has never experienced a cold and has never been hospitalized before. She noted that she does not take any medication. She only takes ibuprofen when on her period. She reported that she takes the medication to manage pain as a result of cramps. The patient denies taking any supplements or prescribed medications. She reported using cigarettes about three times. However, since then, she has never used cigarettes before and does not plan to any tobacco products in the future. She denies taking alcohol. However, she reports that her parents take wine during dinner. She denies medication allergies. However, reported that she is allergic to peanut products. She noted that she was allergic to peanut products at the age of 4. Her mouth starts itching when she takes peanut products. Through the interview, it is noted that the patient is literate on how to manage her allergies. For instance, she reads food ingredients before taking them. She has never taken any strong drugs. Overall, the patient is healthy with no pertinent health problems.
What I Learned
I learned a lot in the shadow health interview. One of the things I learned is that the patient does not have any significant health problems, except for a peanut allergy. I also learned that the patient does not have the urge to engage in alcohol or substance use. I also learned that the patient uses ibuprofen to manage the pain she experiences during menstrual cramps. I also learned the importance of using open-ended questions during patient assessment. Bischof et al. (2021) noted that open-ended questions help nurses gain a comprehensive understanding of the condition of a patient, including social, emotional, and physical aspects, which can lead to a more accurate diagnosis and treatment plan. Open-ended questions also encourage patients to freely express themselves and this allows them to offer relevant and detailed data about their concerns, symptoms, and medical history. I learned how to construct and use both open and close-ended questions to comprehensively assess the patient.
How I Grew
I grew my knowledge of patient assessment. One of the skills I grew is communication. I provided the patient a chance to share her views about her health. Another skill is observation. I constantly observed the patient and ensured that I captured all the symptoms and abnormal appearance. Other skills I have grown in during the shadow health assignment include physical examination, vital signs assessment, pain assessment, and patient history taking. When interviewing the patient, I observed her and assessed her pain.
How I Plan to Improve
I plan to improve my assessment skills by comprehensively interviewing patients often to master the skills. I also plan to improve by attending professional development activities. In the activities, I will learn more about patient assessment. I will also improve by learning from my supervisors at the health facility. I have a lot that they can teach me.
