In this Assignment you are asked to apply knowledge related to referring clients to obtain resources and interventions. Often one community provider alone does not service a person/family in need. It is imperative for you as the professional to be aware of how and when to refer and what resources and interventions are available to the client. For this Assignment you will utilize your knowledge from course readings and investigate further in the Library a program which coordinates referrals and resources.

Part 1

Please audit two programs which coordinate referrals to resources and interventions in the community. Provide a conceptual overview of these two programs.

Part 2

Using one of the two programs chosen in part one; assess how this program will provide required resources to a risk population in need. Please provide a synthesis of the following: the at-risk population, the resources utilized, and how the chosen referral program assisted in accessing the resources by the at-risk population noted.

Part 3

Using one of the two programs chosen, assess how this program will provide required referral to an intervention to serve the at-risk population. Please provide a synthesis of the following: the at-risk population, the interventions utilized, and how the chosen referral program assisted in accessing necessary interventions.

Please note Part 2 and 3 do not have to serve the same at-risk population.

This paper should be 3–5 pages (not including the cover page and reference page) in APA style format. Your paper should include a minimum of three academic peer-reviewed journal articles. Provide appropriate citations and references for any information you use in this paper.
