Change is inevitable in health care. As I reflect on my nursing career I have seen and been involved in a significant number of change projects. I would use Roger’s change theory to support the process of change. There are five stages within the theory that include knowledge, persuasion, decision, implementation, and confirmation (Sullivan, 2013.) Each stage is essential to have meaningful lasting change. The first stage speaks to the need to gather information and establish a good understanding of the issue or problem. At this stage change agents must believe that there is a need for change. The next step addresses the persuasion stage. During this portion the change agent must persuade others of the need for change and the possible “road maps” that will be used to facilitate the change. Then comes the decision stage of the process. At this point the change agent has to make the decision on which plan will be followed to bring about the desired change. The last stages of the theory are the implementation and confirmation. Initiating and carrying out the plan is the implementation stage. The confirmation allows the change agent to establish if desired change has been reached. According to Sullivan (2013), “Rogers stress two important aspects of successful planned change: key people and policy makers must be interested in the innovation and committed to making it happen” (p. 58.) I selected the Rogers theory because it is similar to the process improvement model that I use in my profession. Having the key players within an organization be supportive of change is instrumental in truly reaching meaningful and lasting change.

I believe the theory of choice that I would use to support the process of a major change would be the Lewin’s Change Theory. During his field theory he states, “One’s behavior is related both to one’s personal characteristics and to the social situation in which one finds oneself.” Lewin’s most influential theory is his model of the change process in human systems. Lewin’s theory is a three-stage model of change is known as the unfreezing-change-refreeze model this requires prior learning to be rejected and then replaced. Within his theory he focuses on driving forces that push in the desired direction of change. This theory also speaks of restraining forces, and equilibrium. Following the stages starting with stage one being the unfreezing stage this is where you direct behavior away from the existing situation and move into a new direction of change. Following that is the change part of the theory which is where it involves a process of change in thoughts, feelings, and behavior that liberates and makes one more productive. The final part of this theory is to refreeze and this means to freeze the change that has been made this will create a new habit and now becomes the standard operating procedure. I believe how I could support this theory is to try using it. Working with behaviors and reinforcing a new behavior, by helping others understand the need for a change. Educating why and how, and then leading gently into the change. Considering others thoughts, opinions, feelings and having a deeper understanding of behaviors displayed will help me know how to guide and direct others in the process of a change. Then once the change has been made, reinforce it, continue to educate, and guide and always be available for providing direction.

Change can be difficult for anyone and some nurses are more open to change than others. A great example is when many practices, hospitals and other medical facilities transitioned from paper charts to EMR and advances in their technology. There are surgeons who still complain about the transition even though it has already been two years where I currently work. Nursing management and administrators need to take into consideration different change theories when implementing new programs as they transition to electronic medical records. Lewin’s Theory of Change is one that can be applied to any facility in a variety of nursing specialties. The concept of the theory is to break away from how things have been done in the past and prepare those involved for the upcoming change to take place in the near future. Next it is to move forward with the change providing support, training and expect those who are going to drag their feet. Finally, the change is in place and there is continued support, recognition for those who have transitioned and listen to what is working and what is not so that adjustments can be made to improve the new system of change. Also continue to strive for acceptance and openness with everyone participating in the change while continuing support and communication for a successful transition utilizing the Lewin’s Theory of Change. (Shirey, 2013).
