Case Study: Assessment and Interventions

1. Interview an older adult (60 years or older) who is living with a chronic or terminal

illness or serious disability concerning spiritual status and needs. The older adult could

be family, friend, someone living in a retirement home or someone who is in the hospital.

2. Obtain some basic information from the older adult: Person’s initials, age, sex, ethnicity,

and medical diagnosis. How long have they had the medical diagnosis? Is this person in

the hospital, retirement home, your home; is he/she a family member, friend? How long

have you known this person? What is his/her religion? Do they practice their religion?

If so, how? If not, why not? Do they have accessibility to a minister, chaplain, spiritual

leader of their religion if needed? Do they desire to contact that person? (10 points)

3. Assessment: Complete one of the spiritual Interview an older adult of your choice. These will

be handed out in class. Also complete the Stoll 1979 Assessment tool as well, next page.
