Create an H&P note to submit
Access and complete the Shadow Health Comprehensive Assessment with patient Tina Jones.
Complete each component of the H&P based on the information/interaction with Tina Jones.
Identify 3 or more diagnoses (dx) – primary and additional diagnoses related to existing chronic problems; listed in order of priority. Use the interview transcript
Include a paragraph describing the pathophysiology of the #1 primary diagnosis.
Write a basic treatment plan based on the diagnoses and evidence-based practice (BP). Include two medication prescriptions (may include OTC products).
Follow APA format for document – maximum of 6 pages (not counting title page, references & prescription pages).

Make too sure to review the assignment guidelines and rubric under Week 6. The paper needed to be APA format.
Keep in mind that the contribution of the Subjective information (Medical Hx) is critical to determine the medical dx. The Physical Exam is used to confirm/evaluate the potential dx.
